The pricing listed below is available for small businesses with two or more employees who sign up for the Big Stuff Health Share and direct primary care services. In a nutshell, direct primary care is like having a family doctor on retainer.

What’s included in your membership:

  • Alive & Well plans include the Primary Member/Head of Household.
  • Spouses can be included as long as they are legally married to the Primary Member or listed as a dependent on the Primary’s tax return.
  • Dependent Children can be included on Primary’s membership as long as the Primary is the designated legal guardian of the child. This includes birth, adoption, or foster care.
  • Children age 25 or younger may be added. Children age 26 and older are not eligible for our services and must sign up for their own account. Married children are required to have their own membership.
  • Add $10 per month per additional family member over 8 people.
  • Members must participate in Medicare/Medicaid if Medicare/Medicaid eligible.
  • Pre-existing signs/symptoms/diagnoses/history are not sharable for 12 months after membership start date.
  • Pregnancy is sharable only if conception occurs 90 days or longer after membership start date.

Additional Information:

  • The group pricing listed above is available to groups of 2 or more primary members.
  • The number in your group must represent at least 70 percent of your full-time employees.
  • Alive & Well Complete prices include Alive & Well Direct Primary Care and medical cost sharing. Each year Alive & Well evaluates and pairs itself with what it feels is the best health share to meet the needs of our members. All members will receive a notification of change via email and may opt-out of the changes at any time.
  • Alive & Well Complete plans add an additional $50/member/month for tobacco use.
  • All plans include a one-time enrollment fee of $150 per primary member.
  • All health share payments must be pre-approved by our providers before or during the need.
  • All members must complete a health & personal history questionnaire and an initial Health & Fitness Assessment prior to consideration of membership.
  • All members are subject to membership guidelines for consideration of payment.
  • Inappropriate use of disbursed funds is cause for immediate dismissal.
